Vólvulo de la vesícula biliar
Introducción: el vólvulo de la vesícula biliar es una causa poco común de abdomen agudo, caracterizado por rotación sobre su mesenterio a lo largo del eje longitudinal del pedículo. Se presenta un caso de vólvulo de la vesícula biliar. Presentación de caso: paciente femenina de 71 años de edad quien consultó por presentar dolor de fuerte intensidad, intermitente y súbito en el cuadrante superior derecho abdominal acompañado de náuseas y vómito. El abdomen estaba blando no distendido, con dolor leve a la palpación profunda en el cuadrante superior derecho. La ecografía mostró vesícula biliar distendida y con engrosamiento asimétrico de las paredes, sin evidencia de cálculos. Durante la cirugía estaba negruzca, muy distendida, con zonas de gangrena y necrosis, sin evidencia de perforación, y girada 360º sobre el eje longitudinal en sentido antihorario. Discusión: la manifestación principal del vólvulo de la vesícula biliar es el dolor abdominal en el cuadrante superior derecho, pero sus características son variables, pudiendo imitar a la colecistitis aguda, por lo que puede ser difícil su identificación en el preoperatorio. El diagnóstico definitivo por lo general se realiza en la cirugía. El pronóstico suele ser favorable cuando el diagnóstico es oportuno. Conclusión: el vólvulo de la vesícula biliar es una urgencia quirúrgica poco común que debe sospecharse ante signos y síntomas de colecistitis aguda. La colecistectomía es el tratamiento de elección con excelente pronóstico. Abdomen was soft, not distended, slightly painful on deep palpation in the right upper quadrant. Ultrasonography showed distended gallbladder with asymmetric thickening of the walls, without evidence of gallstones. During surgery gallbladder was found to be blackish, markedly distended with areas of gangrene and necrosis, without evidence of perforation and rotated 360º on the longitudinal axis counterclockwise. Discussion:The main manifestation of gallbladder volvulus is abdominal pain in the right upper quadrant, but its characteristics are variable and can mimic acute cholecystitis, so it can be difficult to identify it preoperatively. Definitive diagnosis is usually made during surgery. Prognosis is usually favorable when the diagnosis is timely. Conclusion: Gallbladder volvulus is an uncommon surgical emergency that should be suspected in all patients presenting with signs and symptoms of acute cholecystitis.
